Community Reintegration Services (CRS)

Community Reintegration Services is a multi-disciplinary team that provides intensive case management to severely and persistently mentally ill adults who live in the community and require additional assistance to maintain their housing.  The team meets with consumers at least weekly to provide assistance in linking clients to needed medical and other community resources, and empowers program participants to reach for and maintain their optimal resiliency, wellness and independence.  Referrals come from the Patch and Premier programs, acute inpatient psychiatric units and sometimes from the San Bernardino County homeless programs

Homeless Outreach Support Team (HOST)

Homeless Outreach Support Team (HOST) provides outreach and engagement services to individuals and their families in an effort to obtain permanent supportive housing. Wrap around services are provided by case managers to assist the individuals to maintain housing and become resilient in the community. Services include outreach, intensive case management, advocacy, and empowerment for self-sufficiency, as well as linkage and referrals. HOST collaborates with the Sheriff’s Homeless Outreach Proactive Enforcement (HOPE) team to conduct outreach events, go out into the field, and engage the most difficult and hard to reach clients.
