Care Coordination

Care Coordination services help navigate medical, educational, social, and other community services to aid a person in their recovery.  Care Coordinators also assist in navigating a person through their course of Substance Use Disorder (SUD) treatment.

Care Coordination services also include:

  • Assistance in developing a plan of reaching goals.
  • Assist with referrals to other services, including food and clothing.
  • Assist in arranging medical and/or dental appointments.
  • Offer other services to assist in reaching goals.
